How can I create an email list signup form for Myspace?

I’m a musician and I would like to have an email list signup on my Myspace.
I already have a signup box, that takes you to another site that I use for my email marketing, where you complete your registration (myspace.com/alinamusic). I would like to have the registration fields (name, age, country etc) on my Myspace page directly like these artists do: myspace.com/christinaaguilera, myspace.com/avrillavigne
What would the HTML codes for this be and how do I make it add the info typed in, to an email list?

Just go to the site and take a form at the javascript or the HTML section of the site. You’ll also need a host that supports PHP, ASP, CGI or other so you can receive the emails.
